For severe scars, surgical excision may be accompanied by layering of triamcinolone, long-term placement of intradermal sutures, and subsequent monthlycorticosteroid administration. Severe scars may alsobenefit from concomitant intralesional 5-FU (0.9 mLof a 50 mg/mL solution) and corticosteroid injections(e.g., 0.1 mL of triamcinolone acetonide 40 mg/mL)administered monthly or treatment with newer agents,such as bleomycin or mitomycin C.
