Surgical interventions including microvascular decompression (MVD), percutaneous trigeminal rhizotomy, and gamma knife radiosurgery (GKRS) may be effective, but each has its drawbacks. Among all the surgical interventions, MVD remains the best approach in patients when MRI shows a loop of the aberrant vessel. But MVD is the most invasive procedure and contraindicated for elderly patients with multiple comorbidities, the surgery may result in cerebrospinal fluid leaks, cerebral infarction or hematoma and death.
