When a patient is admitted to a hospital, the nurse should take the patient’s vital signs. First, the patient should change out of his or her clothes. After the patient changes into a hospital gown, ask him or her to sit for about 5 minutes before taking any measurements so as to make sure the values are accurate .During this time ,the nurse could take a minute or so to observe the patient and make a general judgment .Does the patient seem anxious, pained, or upset ?
