Starting with the right-side BSSO, an incision is per- formed in the vestibular sulcus from the ascending ramus up to the second premolar down to the bone. Subsequently, subperiostal dissection is performed in order to expose the anterior aspect of the ramus and the posterior body of the mandible down to the inferior border. After adequate exposure has been accomplished, three bone cuts are performed by using the surgical bur, reciprocating saw, and/or piezosurgery. The first cut is made horizontally through the medial cortex of the ramus just above the level of the mandibular occlusal plane and does not exceed 2 cm in length (i.e., a Hunsuck modification).
