MMA surgery plays an important role in the correction of OSA that is refractive to noninvasive therapies. Since large advancements of the maxillomandibular complex are required in medically compromised patients, MMA surgery in OSA patients is generally more complex than “conventional” orthognathic surgery. With proper precautions it is however a safe and highly effective treatment modality for OSA. MMA surgery is probably the most effective surgical intervention in patients who are skeletally compromised (e.g., severe malocclusions, retrognathia, or bimaxillary retrusion). These patients, especially when they are relatively young, should therefore be informed of MMA surgery as primary surgical treatment modality.
